that are not a covered benefit, call UMC at 1-800-479-5122 prompt
#4 for authorization.
- UMC
may ask for further information on an individual basis for those
requests for services not covered by BHP.
- Requests
for authorizations on services that are not a covered benefit and
are not Medically Necessary will be sent directly to the
President for review and determination on an individual basis.
- The
President will review each case and contact the UMC with a decision.
- Requests

Director for review and determination on an individual basis.
- The
Medical Director will review each case and contact the UMC with
a decision.
- The
UMC will contact the requesting provider with the Medical Director
and/or President's decision with one business day.
- Providers
may appeal a denied request for authorization.
